FAMILY WELL BEING

The Family Well Being Team strives to provide Aboriginal children and families with the necessary support services and advocacy they need to achieve a healthy quality of life. Case Managers work in a traditional and holistic way to address the needs of families, and Developmental Service Workers work with families affected by developmental delays and FASD.

Counselling and related programs combine the best of Aboriginal and contemporary approaches to healing and treatment.

Services

  • Ongoing case management

  • Informal, supportive and crisis counselling

  • Complete psycho-social assessments and service plans

  • Attendance at initial, review, transfer and case conferences

  • Advocacy, mediation and support with social service agencies

  • Support in the home and community

  • Personal support for clients involved with Family or Criminal Court

  • Connecting clients to Elders, ceremonies, teachings and community events

Referrals

We provide referrals to internal and external Aboriginal community resources, such as:

  • Addiction and treatment programs

  • Food, furniture and clothing banks

  • Housing providers such as Wigwamen and Amik

  • Other social service agencies

Advocacy and mediation

We provide advocacy and mediation services with government agencies and other organizations, such as:

  • Toronto District School Board

    • We attend Individual Planning Review Committee and Individual Education Plan meetings.

  • Ontario Works and Ontario Disability Support Program

    • We work to ensure our clients receive all financial rights and entitlements, including transportation allowances and daycare.

  • Housing Connections and other housing providers
